Our community habilitation service provides support to help adults with intellectual and developmental disabilities live rich and meaningful lives.
LAUNCH’s Community Habilitation (Com Hab) program provides clients with access to community activities and resources, nurturing skills specific to each individual’s personal goals. Our counselors understand that everyone has strengths, which we carefully help cultivate at home and in the community. Areas of development include:
- Learn skills to live more independently:
- Cooking
- Housekeeping
- Grocery shopping
- Money management and budgeting
- Learn how to be more active in your community:
- Build friendships both interpersonally and within the community
- Access city bus and ride share options
- Social skill development
- Self-advocacy
- Volunteering
How Can You Become Part of This Great Community?
Individuals must meet eligibility criteria as defined by the New York State Office of People with Developmental Disabilities (OPWDD), be eligible for Medicaid, and be enrolled in the Home and Community Based Services Waiver (HCBS). Referrals are made through the Care Manager.
